Things to check are:

Q - Have non-alphanumeric characters (eg. . [] () {} / \ ! @ # $ %^ &*) been used in the image filename?

SOLUTION: Please rename the image using use alpha and numeric characters. Note: Hyphens and underscores can also be used (e.g. file_name-01.jpg). The, re-upload the image and save your changes.
